Lead Selection
It is important to select the correct leads that display the greatest amplitude and clearest signal in order for the monitor to accurately detect QRS complexes and R-waves. When selecting leads, consider the following guidelines:
The QRS complex should be tall and narrow (recommended amplitude > 0.5 mV).
- R waves should be above or below baseline (but not biphasic).
- The P wave should be less than (1/5) the height of the R wave.
- The T wave should be less than (1/3) the height of the R wave.
To avoid detection of P-waves or background noise (e.g. QRS complexes), the minimum level for detection of QRS complexes according to specification AAMI-EC 13 is set at 0.15 mV. When the ECG signal is too weak, it may be a false positive for heart disease. The arrest is visible.
For many monitors, increasing the gain (amplitude) of the ECG does not change the monitor signal. Therefore, the signal must meet the above criteria without changing gain in order for the monitor to accurately analyze the waveform.
